Pascal’s Risk Trigger® Monitor (RTM) software scans every electronic medical record using the industry’s most advanced trigger library to identify potential harm and risk events using real-time EHR data.
2. Verify & Notify
Our Expert Clinical Review (ECR) teams bring decades of front line experience to our client hospital and health system partners. These patient safety specialists are able to dive deep into every aspect of the patient’s record to conduct “sense-making” if appropriate and escalate for action. Our ECR teams are clinically integrated with every client facility 365 days per year.
When an adverse event is clinically validated, VPS uses a standardized process to document and classify the harm across dozens of dimensions, including severity, preventability, possible causes, and related factors (such as medication, procedure, device, and more), and the client stakeholders are notified. In some cases, we’re able to notify our clients of the potential for harm before it occurs.
3. Analyze and Guide
We don’t just tell you what, when, how, and why the harm is happening. Pascal’s VPS also includes deeper analysis and insights by our Expert Safety Advisory (ESA) teams, on patterns, trends, outliers, and much more. We proactively partner with our clinical clients to help create plans to reduce harms across a single hospital, or an entire health system.
